How GLP-1 savings cards work

Manufacturer savings cards (also called copay cards) are programs from Eli Lilly (Mounjaro, Zepbound) and Novo Nordisk (Wegovy, Ozempic, Saxenda) that reduce out-of-pocket costs for eligible patients. They are not insurance, and they do not lower the list price. They reduce what you pay at the pharmacy counter, with the manufacturer covering the difference up to a savings cap.

Who qualifies

You must have commercial insurance, meaning coverage through an employer or the individual marketplace. Federal law (the Anti-Kickback Statute) prohibits the use of manufacturer savings cards for patients on Medicare, Medicaid, VA, or TriCare. Whether the drug is on your formulary affects how much you save: covered drugs typically reach $25/month copays; non-covered drugs see $400 to $500/month in savings off list price.

How to apply

Visit the manufacturer's savings page (linked in our sources below), confirm eligibility, and either download a card to bring to your pharmacy or have the pharmacy apply it directly. Most cards are issued instantly. The pharmacy enters the BIN, PCN, Group, and ID numbers from the card alongside your insurance information when filling the prescription.

Common pitfalls

  • Annual savings caps mean you may exhaust the benefit if your insurance has a high deductible.
  • Cards must be renewed each year. Last year's card may not work on next year's fill.
  • Switching pharmacies sometimes requires re-applying the card or providing the BIN/PCN/Group/ID again.
  • If you change from commercial to government insurance mid-year, the card stops working immediately.

What to do if you do not qualify for a savings card

  • Medicare beneficiaries. Check if your Part D plan covers the drug. Some plans now have GLP-1 coverage with Tier 3 or Tier 4 copays. As of 2026, Medicare Part D covers Wegovy when prescribed for cardiovascular risk reduction in patients with obesity and established cardiovascular disease.
  • Uninsured patients. Apply for manufacturer patient assistance programs (NovoCare for Novo Nordisk drugs, Lilly Cares for Eli Lilly drugs). Lilly Direct also offers Zepbound vials at approximately $499/month, which bypasses the pharmacy supply chain.
  • Cost-sensitive patients. Compounded semaglutide or tirzepatide from licensed compounding pharmacies typically run $200 to $400/month. Verify the pharmacy is licensed in your state and the FDA does not currently flag the drug as resolved-shortage (compounding is restricted when shortages end).
  • VA patients. The VA formulary covers semaglutide and tirzepatide for eligible diabetes and weight management indications. Speak with your VA primary care provider.
